【リスニングテスト#6】初診料について説明する英文の解説
日本では総合病院にかかる時に紹介状が無い場合は初診料がかかることが一般的です。そのシステムを簡単に外国人に口頭で説明しているシチュエーションを想像してください。同意書のような硬い表現とは異なります。

➡ There is an extra fee that should be charged for your first visit consultation, which is 3000 yen.
追加でかかる料金を extraという形容詞をつかって説明しています。本来の量や数、または期待や予期される量や数より多いことを表します。使われるシチュエーションによって、プラスなイメージにもマイナスなイメージにもなります。例えば、
✔ Can I have extra bags? (袋を余分に（＝追加で）もう数枚いただけますか？)
✔ Can I have extra Wasabi? （ワサビをもうすこしいただけますか？）
✔ I have extra cash on me to buy this antique dish.（このアンティークのお皿を買う余分な現金を持ってるよ！）
✔ I can’t believe it! They are expected to put in extra hours without pay.（信じられない！彼らは来週、無償で数時間余分に働くことになってるって。）
➡ Unfortunately, this fee is not covered by your national health insurance plan. 残念ながら、この料金は国民健康保険ではカバーされないんです。
再び受け身の文です。coverという動詞を使って、保険が支払うことを表します。この文は Your national health insurance plan doesn’t cover this fee. と表してもOK。
➡ Would you still like to make an appointment? それでもご予約されますか？
still：余分に３０００円かかるけれども、「それでも尚」という雰囲気を加えます。相手の返答がNOかもしれないと予測される場面などでよく使われます。
予約はreservationやappointment という訳がありますが、病院ではappointment が使われることが一般的でしょう。
